PeopleSoft Signature Page
The PeopleSoft Signature Page is required for every new proposal that requires an institutional signature. Additionally, a printed copy with appropriate signatures (Deans or their representatives, Chairs or their representatives) must accompany electronic submission. If you have UMKC co-investigators, their Dean(s) and Chair(s) need to sign the PeopleSoft Signature Page as well.
PeopleSoft Signature Pages will no longer be required for "noncompetitive continuation" applications; ORS will, however, require a line item budget that has been approved by the Fiscal Officer, as indicated by initials or stamp on the budget page or an attached document. If you have questions or need assistance determining if your proposal constitutes a noncompeting continuation, please contact your Fiscal Officer.
General Financial Disclosure
(Fill out on-line and print) All key personnel (including subcontracted personnel) should complete this form and submit it to ORS when you submit your application and PeopleSoft Signature Page.
Sponsored Programs Space Commitment Form
(Fill out on-line and print) The space form is no longer required with all proposal submissions. Complete the form only if you will need additional or modified space to complete your proposed project. If you do not submit a space form, ORS and the Physical Facilities Committee will assume no additional space is needed for your project.

PHS 398
As of May 10, 2006 the 9/04 forms will no longer be accepted. The 4/06 forms will be required.

08/16/02: NIH As of June 30, 2002, NIH will no longer mail out paper copies of the PHS 2590 face page to alert investigators that their progress reports (also known as non-competing continuation applications) are due. Instead Sponsored Programs & Research Support will alert faculty of progress report due dates by e-mail.

Competing or New Awards
1. Completed PeopleSoft Signature Page
a. Signed by PI, PI's Chair, and PI's Dean
b. Signed by UMKC Co-Is, or others receiving shared credit and their Chair(s) and Dean(s)2. A completed Financial Disclosure Form for all key personnel.
3. Detailed budget (regardless if a detailed budget is required by the funding agency)
4. Copy or electronic access to the completed proposal and application forms
5. Subcontract proposal documentation (if you have proposed a subcontract to another)
6. Space Form (Only necessary if you are requesting additional space or renovating existing university space)
Noncompeting Continuations
1. No PeopleSoft Signature Page necessary
2. Detailed budget with an indication that the Fiscal Officer has approved the budget (for example, the fiscal officer has initialized or signed the budget)
3. Copy or electronic access to the completed proposal/progress report and application forms
4. A completed Financial Disclosure Form for all key personnel
5. A signed Non-Competing Continuation Certification Statement
Please notify the Office of Research Services of your upcoming proposal submission well ahead of the deadline date. Even if the submission date is far in the future, as soon as you know you'll submit, please contact ORS.
Often agencies require PIs to submit preliminary proposals before full-proposals will be accepted. If you are submitting a preliminary or pre-proposal that requires an authorized university official's signature (for example, if your preliminary proposal must be submitted via NSF's FastLane), you will also need to submit a PeopleSoft Signature Page before submission. Other forms may be required (depending on the grant and preliminary proposal guidelines). Contact ORS (235-5600) if you're planning to submit a pre-proposal, and we'll let you know what internal forms will be required.
Submitting Subcontracts (UMKC as subcontract)
When UMKC is the proposed subcontractor to another organization's proposal, UMKC institutional authorization is still necessary at the time of application. If the lead institution does not specify a letter format or form they would like subcontracting organizations to utilize, please utilize the following:

Grant programs that restrict the number of proposals by institution
Some grant programs require an institution to submit only one proposal per competition. Likewise, administrators of fellowships or other individual grants might ask that an institution nominate only one or two faculty members for a fellowship opportunity. If you are interested in applying to a program that limits the number of proposals by institution, please complete the following "intent to apply" form, and e-mail the form to Linda Daugherty daughertyl@umkc.edu.
