Student or General Public Injury or Property Damage
1. Call the Campus Police, as necessary, at ext. 1515 from a campus phone for accidents or incidents involving students or the general public.
2. Call UMKC Risk Management Office at ext. 1623 and report all injuries.
3. Encourage an injured individual to seek needed medical attention. Do not suggest that the University will take care of the cost of his/her medical care.
4. Complete a UMUW Form 200 "Student or General Public Injury and Property Damage Report ;"
* If you witnessed an accident or incident or an accident or incident was reported to you.
* If you are in charge of or are sponsoring an activity during which an accident or incident took place;
* If you are in charge of a building or facility in which an accident or incident took place;
5. Be sure to obtain the name, telephone number and address of individuals who have sustained bodily injury, allege personal injury, or who allege property damage or loss. Get names, telephone numbers and addresses of all witnesses.
6. Within twenty-four hours (24) of the incident, forward the typed completed UMUW Form 200 Student or General Public Injury or Property Damage Report to UMKC Risk Management, 4747 Troost, Rm. 23. Do not wait until you receive copies of a Police Report or other documentation before sending the Form 200 to Risk Management.
