Vehicle Accidents

1.    Call appropriate police authority if a vehicle, either owned or leased by the University, or an employee vehicle if used for official University business, is involved in an accident.

2.    Drivers who hold a commercial driver's license (CDL) should call their supervisor immediately.

3.    At the scene, discuss details of the accident with law enforcement officers only.

4.    Exchange vehicle license, operator license, and insurance information with other involved drivers.

5.    Record year, make and model of other involved vehicle(s).

6.    Obtain names, telephone numbers and addresses of any potential witnesses.

7.    Make a sketch of the scene for later reference.

8.    Note the visible extent of damage to other vehicle(s).

9.    Obtain a police report reference number.

10.    Driver should complete sections 1 through 40 of a UMUW Form 5 "Vehicle Accident Report."

11.    Supervisor should complete the section titled "Supervisor's Report of Accident Investigation" on the back of the UMUW Form 5.

12.    Within twenty-four (24) hours, forward a completed typed UMUW Form 5 to Risk Management, 4747 Troost, Rm. 23. Attach any supporting documentation (police report number, copies of citations, etc.).
