Do I Change my W-4 or Tax Withholding 
You have your choice of three ways.

You can
go to the MIS Web Applications site and change your restriction code.
Accessing the Web Applications is explained at the site itself

The form is NOT an on-line form that downloads information to this
office. You must print it off, sign it and forward to AC 226
for processing.

Once Form is Completed, Sign the Form and send to:
Human Resources
AC 226

Obtain a W-4 form from your departmental payroll contact or from HR. Human Resources will mail you a blank form if you call 1521 and request. Completed form needs to be returned to Human Resources AC 226.
