Patient Assistance Program for HVP Vaccine

UMKC is pleased to offer Gardasil, the HPV vaccine to our female students.  The vaccine is a three shot series at a cost of $140 each.  That is a total cost of $420 for the entire series.

Student Health and Wellness is well aware of this being cost prohibitive for some of our students.  Therefore, we are working with Gardasil’s manufacturer, Merck to provide financial assistance for those student’s that qualify.

If you are:  uninsured or have insurance that does not cover vaccines; age 19 or older; have an annual household income less than $20,240 for individuals, $27,380 for couples, or $41,300 for a family of four then you may be eligible for Merck’s patient assistance program.  Merck recognizes that sometimes exceptions need to be made based on a patient’s individual circumstances.  Individuals who do not meet these criteria may still qualify if they have special circumstances of financial or medical hardship.

To apply for assistance, please print application form, fill out and return to UMKC Student Health and Wellness or you may fax to the clinic at 816-235-6565.  We will submit applications once a week and will call you back with the results.
