Charges in relation to lost or stolen cards:

All cardholders pay a fee for the replacement of lost or stolen cards. Replacement card charges are $10 for the first card, $15 for a second card and $20 for all other cards.

 

Charges in relation to damaged cards:

All cardholders pay a fee for the replacement of damaged cards. The charge for a replacement card is $3 if the cardholder turns in their damaged card when a replacement card is being issued.

If the cardholder does not turn in the damaged card when a replacement card is being issued, charges associated with lost or stolen cards will apply (please see above for additional details).

 

Changing the name/picture on an existing card:

Requests for a name change or new picture, resulting in the issuance of a replacement card, costs $3 if the cardholder turns in their old card when a replacement card is being issued.

If the cardholder does not turn in the old card when a replacement card is being issued, charges associated with lost or stolen cards will apply (please see above for additional details).

If it is determined that a name is incorrect due to the University's error, no charge will be assessed to the cardholder for a replacement card as long as the old card is turned in when the replacement card is issued.
