Student can change their official name on file with the university by completing
a student information change form and supplying appropriate documentation to the
Records Office.
Acceptable forms of identification include:
- a current
drivers license
- court documents such as marriage licenses and divorce decrees
- original social security cards
- other identification as approved by the Registrars Office
