Application for Possession and Use of Radiation Sources

Credit Card Approval For Radioactive Material

(documents can be submitted to the RSO or the RSC Coordinator via e-mail)

Mandatory Items to be completed prior to RSC approval:

·    Completion of the Office of Radiation Safety’s on-line training,

·    Completion of a 3 hour lab/Demo session with the Health Physics staff, and

·    An observation by one of the Health Physicists the first time the AU uses radioactive materials in the laboratory.

Amendment Form

Amendment To Authorization For Possession And Use Of Radiation Sources

(documents can be submitted to the RSO or the RSC Administrator via e-mail)

In order to initiate new work under an existing approval, the investigator should contact RSO, Susan Masih, to determine if an amendment to
the application is necessary.  

Renewal Form

 Renewal for Possession and Use of Radiation Sources

(documents can be submitted to the RSO or the RSC Administrator via e-mail)

After three years, the principal investigator must submit a new application if the research is to continue. If the research will not continue, the PI needs to notify the RSC office.     

Research Involving Animals

• Approval of research using radioactive materials with animals must be received by both the UMKC RSO/RSC Coordinator and IACUC before animal use can begin.
• The IACUC is the Chancellor's advisory committee responsible for oversight and evaluation of all animal care and use on the UMKC campus.
