Forms

Please note that all forms submitted to the Office of Student Involvement must be typed.  In addition, many of our forms must be submitted at least two weeks prior to your event, so please plan accordingly. 

Starting February 1st, 2011 all forms must be delivered to the Office of Student Involvement in electronic format.  These forms should be typed, scanned, and emailed to getinvolved@umkc.edu at least two weeks in advance.  No forms will be accepted via fax or hard copy.

 

On this page, students can find a complete listing of all the forms that student organizations may need for their various functions at UMKC. If you have questions about any of the forms, please e-mail us.

 

Application for Student Organization Recognition

This form will tell you how to start a new student organization at UMKC. Please also read the "How to create a constitution for student organizations" form.

Area/Equipment Request Form

This form should be completed for any event or activity that will take place outdoors.

Dance/Social Event Form

This form should be completed for any dance or social event that is scheduled on campus. Please also review the Dance/Social Event Policy.

Demonstration/Rally Permit Request Form

This form should be completed when your organization would like to hold a demonstration or rally.

Facilities Request Form

This form should be completed to reserve any room or space in a University building excluding the University Center. To reserve space in the University Center, call 816-235-1416.

Permit to Sell

This form should be completed whenever there will be cash exchanged for a product or service.

Poster Request Form

Enlarge your 8" x 11" fliers to banner size. Cost: $10 each. Use this form to submit your request.

Sound System Check Out Form

Complete this form to reserve a sound system for use at your student organization event in the University Center or University Playhouse/University Center Walkway. The large sound system comes with two large speakers, two CD players, two tape decks, one wireless microphone and requires two operators from your organization. The small system comes with one CD player and two wireless microphones. Please also see the Sound System Usage Policy.

Student Organization Listserv Request Form

Complete this form to set up a listserv for your student organization, or to make changes to an existing listserv. Please print and submit form for signature approval to the Office of Student Involvement.

This form must be printed off and delivered to the Office of Student Involvement for signature.  Please do not submit this form directly to Information Services without appropriate OSI approval. 

Student Organization Webpage Request Form

Complete this form to set up a Web site or e-mail address for your student organization, or to make changes in who may access the Web site or e-mail address. Please print and submit form for signature approval to the Student Life Office.

This form must be printed off and delivered to the Student Life Office for signature.  Please do not submit this form directly to Information Services without appropriate SLO approval.
